Preparing For Your Visit

Whether this is your first time visiting us or your first time meeting with a mental health provider, welcome! Atlanta Wellness Collective provides a calming and approachable, yet professional atmosphere so you can look forward to your appointments.

Prior to your initial appointment, please fill out your new client forms at least 24 hours prior to your appointment. New client forms will be emailed to you through your Client Portal once your appointment is confirmed. For anyone under 18 years old, an initial parent appointment is required prior to meeting with your child or teen. We look forward to seeing you!

Cancellation Policy

Please notify your provider at least 24 hours prior to your appointment to cancel or reschedule. Otherwise a late cancellation fee will be charged. You can also reschedule through your Client Portal. For Monday appointments, we ask that you cancel or reschedule by Friday of the week prior.
